I’ve been having this same issue since the weekend with a WageWorks card. Removing and re-adding the card didn’t work either. I’m getting a text message from WageWorks that says the transaction was authorized which makes me think this is an issue on the MTA/OMNY side. I don’t understand how it could be broken for three straight days though. The secondary card is a ho-hum no-annual-fee card. It does seem very random, and I've used nearly every not-in-a-rush subway trip to tease out the edge cases of this problem.
